Once I receive the referral, I will contact you to request the patient’s most recent progress notes, laboratory results, and medication list through a HIPAA-compliant method. I will also reach out to the patient directly to schedule their Discovery Call, ensuring a smooth and timely transition of care.
We Accept Insurance! We currently accept Aetna and UnitedHealthcare for our California patients for Nutrition Consult/Medical Nutrition Therapy. Nutrition coaching program The Heart Health Blueprint is not covered by insurance
